Home health aides and homemakers can assist you with personal care and household tasks such as meal preparation, cleaning, and laundry. Our team members can run errands and grocery shop on behalf of our clients. Our team can also provide companionship or supervision for clients that may need more hands-on assistance at home.
Staff provides physical assistance and cueing with personal tasks such as dressing, bathing, grooming, ambulation, and transfers. Participation with all activities is encouraged to maintain the highest level of independence possible. Services are provided with the goal of maintaining a clients dignity and the ability to remain safely in their home. All staff are certified, bonded and have passed background checks.
Staff are available from 1 hour a day to 24 hours, whatever meets the needs of our clients. Short and long term respite services are available.
Clients can use Capuano’s transportation service to go to doctor appointments, to pick up medications, or to go shopping. Whatever your needs may be, our trained team members we will get you there and back assisting you as necessary.
Our care management team assists clients getting the services they need to remain safely in their home. The team consists of a social worker, registered nurse, home health aide, and physical therapist. A licensed team member will complete a comprehensive assessment in your home, make recommendations, and coordinate services with you. With the clients’ permission, team member coordinating your care can maintain contact with family, friends, or legal representatives, to ensure they are aware and are able to provide input with the plan of care.
Designed to assist elders and their families obtain the services necessary to remain safe and as independent as possible in their home environment. Our team will then design the short and long term plan tailored to the individual 's need.
Our team members are certified by the Alzheimer’s Association to train and assist caregivers. We assist with referrals to appropriate agencies for additional services, day programs, and alternate living arrangements.
Interventions sometimes require the elders and their family make difficult lifestyle changes. CAPUANO CARE’s Elder Care Management Team will be there to assist you in making informed decisions and offer support during every challenge.
The “WATFY” program provides 4 hours of care for someone who is returning home after a hospital or rehabilitation stay. Trained team members are there for you when you are discharged and transport you home. They are able to pick up prescriptions, go grocery shopping, and prepare meals. Our goal is to make sure our clients are comfortable and well-situated before finishing our services.
Staff is dedicated to providing all in home services needed to keep you healthy and safe in your home. From nursing and therapists to personal care assistance, homemaking and transportation. We are there when you need.
Homemakers are paraprofessionals who work in the clients home providing support, supervision, companionship and performs light housekeeping. They are able to run errands including grocery shopping, picking up medication. Aides are supervised by a registered nurse. All staff have taken a homemaker course, are bonded and have passed background checks.
Home Support Staff can be scheduled from one to twenty four hours a day. The intake department can assist you in determining what staff can best manage your care.
