Psychiatric Nursing

Capuano Care has experienced, Medicare-certified, psychiatric care nurses to provide skilled nursing care for clients with psychiatric illnesses, memory impairment, or behavioral problems. We believe clients live more fulfilled lives with a structured nursing team by their side.

Breast Care Post-Mastectomy

After any breast cancer surgical procedure, our nurses can provide specialized care for the individual in their time of need. Whether you need wound care, drain care, or an On-Q pain pump removed, we have nurses to help with it all!

Phlebotomy

With a physician's order, Capuano Care nurses are able to skillfully draw blood specimens at home if a patient is unable to get to an outpatient laboratory.

Community Elder Care Management

Capuano Care nurses provide case management services to prevent frequent or avoidable hospitalizations or nursing home stays. Any additional disciplines such as physical therapy, occupational therapy, home health aide, and social work can be added based on a client’s needs.

Physical Therapy & Occupational Therapy

Capuano Care can provide the skilled services of licensed Physical and Occupational Therapists to design and complete a short-term therapy program. The therapists help to assist in recovery from surgery, injury, acute medical incident or a chronic injury.

Visiting Nurse Services

Medication Management

Personalized medication regimen for the individual! Each nurse at Capuano Home Care will help you to establish guidelines for the safe, accurate and appropriate administration and assist with management of medications.

Habilitation Therapy

Our team consists of a Social Worker, Registered Nurse, physical therapist and personal care aides, they are trained to assist persons with advancing Alzheimer’s disease or dementia to remain safely in their home. The team will complete a comprehensive assessment in the home, identify any Medical, social, cognitive and environmental concerns, then develop an individualized plan of care. Our team will assist in identifying resources and making the appropriate referrals. We will assign a case manager to coordinate the care to the client and family throughout the process.
