Personalized medication regimen for the individual! Each nurse at Capuano Home Care will help you to establish guidelines for the safe, accurate and appropriate administration and assist with management of medications.
Our team consists of a Social Worker, Registered Nurse, physical therapist and personal care aides, they are trained to assist persons with advancing Alzheimer’s disease or dementia to remain safely in their home. The team will complete a comprehensive assessment in the home, identify any Medical, social, cognitive and environmental concerns, then develop an individualized plan of care. Our team will assist in identifying resources and making the appropriate referrals. We will assign a case manager to coordinate the care to the client and family throughout the process.
